FERPA - Authorization to Release Information

Relevant to: Students, Faculty, and Staff.

Introduction

For students who need the help of a parent/guardian, family member, or third party in managing their business here as a student, SUNY Empire requires that students give their permission for the university to release information to those other than the student.

FERPA, the Family Educational Rights and Privacy Act of 1974, is a federal law that pertains to the release of and access to educational records, and applies to all schools that receive funds under an applicable program of the U.S. Department of Education.

FERPA applies to personally identifiable information in educational records. This includes items such as the student's name, names of family members, addresses, personal identifiers such as Social Security numbers, and personal characteristics or other information that make the student's identity easily traceable.

Educational records are all records that contain information directly related to a student and are maintained by an educational agency or institution, or by a party acting on its behalf.

Some students may request that an authorized individual, such as a family member, assist them with information pertaining to their records. The student must:

  1. Complete an Authorization to Release Information Form
  2. Submit signed form to the Registrar’s Office by fax, email or mail.

    1. Fax: (518) 580-0105
    2. Email: RegistrarsOffice@sunyempire.edu
    3. Mail:

Registrar's Office

111 West Ave.

Saratoga Springs, NY 12866

Once processed, faculty and staff at SUNY Empire State University will have your consent to disclose educational records to the designated party.
