How to Locate the Repeat Study Form

Relevant to: Students, Faculty, Staff.

 

 

Introduction

For students who want approval to replace the grade of a previous course taken before the Fall of 2018. Note: when you complete the repeat course and the repeat course form, the more recent grade will replace the original.

 


 

 

Log in to MySUNYEmpire.

 

On the Student home page, go to the upper menu row and click Site Map.

Click on Forms and Publications.

Under Student Quicklinks, Select Registrar Forms and Services.

Here, everything is alphabetized. Scroll down and click on Repeat Study Approval Form.

Select either Undergraduate Student Form or Graduate Student Form.

 

Undergraduate students, print the Undergraduate Student Repeat Course Form, complete, and send to the Office of the Registrar by mail to:

Graduate students, print the Graduate Student Repeat Course Form, complete, and send to School for Graduate Studies by FAX (518) 587-9760, or by email Grad.Services@sunyempire.edu.

 

Nursing Students, print the Graduate Student Repeat Course Form, complete, and send to School of Nursing by FAX (518) 587-5126, or by email Nursing.Program@sunyempire.edu.
