Technology Request Form
SUNY Empire ITS has moved to a new form for the creation of employee accounts. This form will also make the Requests for a computer, phone, and Brightspace access if needed.
Other Requests, such as Banner and software can be made at the Service Request Page
Please Note: it does take between 2-3 business days for the account to be finalized. Calling the Service Desk will not speed this process up.
The new employee will receive their user name the method to set up their email account in an email sent to their personal email about 48 hours before their expected hire date from the HR paperwork.
The Technology Request Form
You can find the form by navigating to the Technology Request Form
Please note, you will need to have submitted the appropriate forms to HR for the new employee to be loaded into SUNY HR before you can start this form.
The first part of the form includes a drop down box to fill in the new employee's name. If you do not find the new employee in this field, you cannot continue. Once you put the employee's name in that field, you must type in the name of the employee in the second field. Check for spelling.
You will also need to add the employee's work address and room number. If they are an Adjunct, you can just fill in Adjunct.
The Anticipated Start Date field assists the technicians with their tasks. It does not speed the creation of the account

In the Primary SUNY Empire Contact Person field, add the person who will get also get notified as tickets and tasks are created for the new account.
In the SUNY Empire Location Working With field, this is also a drop down for the new employee. If they are an Adjunct or fully remote employee, choose Off Site as the location.
In the Additional Comments box, please add anything that would be useful for the new employee's account. This would include things like needing access to CRM Advise, Advance, or Recruit.
