Introduction
Beginning Summer 2022, all military-affiliated students (Active-Duty, National Guard, Reserve, Veteran service members, as well as, their Spouses and Child Dependents) will need to submit an online military request form prior to registration, each term.
This will ensure we have the most current up-to-date military information, including branch, status and funding information on file.
If using VA benefits, completing this form authorizes Empire State University to certify your enrollment for the specified term to the VA.
If not completed, we will not be able to certify your enrollment to the VA or credit your student account.

· Please fill in Percentage, Days/Months Remaining of Entitlement, If Known.

If your VA Benefits were used previously at a different Institution, select “Used VA Benefits at Another Institution”.
*Note: If you used VA Benefits at a different institution, you will need to Submit VA 22-1995 (Post 9/11)/5295 (Fry) Request for Change of Program or Place of Training Form.
If you’re using an additional funding source, click on “Add another funding source" and follow the same steps.
