The following article presents students with information on how to satisfy the Documentation of Disability Status regarding their financial aid.
Discharge Status Form
If the National Student Loan Data system has indicated that you had a prior student loan or TEACH grant discharged due to disability or you have recently applied for a discharge, then you must complete and submit the Discharge Status Form before the financial aid office can determine your eligibility for aid.
