Unusual Circumstances Form
The information contained on this page is specific to Empire State University students.
The Office of Financial Aid will request that you complete this form if you are unable to obtain parent information on the FAFSA and you indicated on your FAFSA that you meet one of the provisionally independent circumstances that the U.S. Department of Education allows.
How to Apply
To complete and submit the Unusual Circumstances Form, you may access the Financial Aid Forms Link () webpage.

- If you prefer to use standard mail, e-mail the Financial Aid office (FinancialAid@sunyempire.edu) to obtain a form and instructions. Please specify the name of the form in your e-mail message.
