Introduction
Additional forms are sometimes requested based on FAFSA results. Please complete the appropriate form only if instructed to do so by the Office of Financial Aid. These forms can include the verification form.
- You must login to the Student Home page to view and fill out the form.
- Please note: Your parent(s) information must be included since you are a dependent student.
- If you already have a form in progress or wish to view a completed form, select it from the list.
- Note: You cannot make changes to a form once you have submitted it. If you would like the financial aid office to make a change to your form on your behalf, please email the item you want corrected and the correct information to financialaid@sunyempire.edu.
