This form is used to request a new project that requires the services of an ITS Project Manager. The information provided in this form will be forwarded to the Chief Information Officer and Vice President for Integrated Technologies for evaluation and follow-up. Requests are reviewed weekly, and initial feedback will be provided within one week.
